Our Catheter Supply Company Accepts

• Medicare - Participating Provider and we accept Medicare assignment
• Private Insurance/Major Medical/Group Health Plans: Accept most plans in all 50 states with the exception of HMO's.

Upon receipt and verification of private insurance or Medicare benefits, Support Plus Medical will contact you to let you know if our catheter supply company works with your insurance plan and if there will be any cost to you. Please note: since we are a provider with most insurance plans, the majority of our patients have no out of pocket expenses at all! Upon receipt of the completed Medical Authorization form and the Physician's Order from your doctor we will ship your first supply to your home. Following, Support Plus Medical will contact you once every few days to discuss your urological supply needs before shipping your next order.

Catheter Supplies Provided:

The types of catheter supplies that will be sent to you are based on the urological supplies requested, your physician's consent and your insurance coverage. If there are catheter supplies requested that are not covered by your insurance, you will be contacted by our office prior to any catheter supplies being shipped.

Intermittent Catheters

Recent changes in Medicare now make it possible for Support Plus Medical (SPM) to supply you with up to 200 intermittent catheters per month. Don't risk infection by cleaning and re‐using your used intermittent catheters any longer.
