Upon receipt and verification of private insurance or Medicare benefits, Support Plus Medical will contact you to let you know if our medical supply company works with your insurance plan and if there will be any cost to you. Please note: since we are a provider with most insurance plans, the majority of our patients have no out of pocket expenses at all! Upon receipt of the completed Medical Authorization form and the Physician's Order from your doctor we will ship your first 90-day supply to your home. Following, Support Plus Medical will contact you once every 90 days to discuss your diabetic medical supply needs before shipping your next order. We provide personalized customer service to our customers and reduce the costs to buy catheter supplies. Help is always there when you need it. We ensure your complete satisfaction.
